Before Creation Engine was used Gamebryo was the engine of choice.
Gamebyro, formerly known as Netimmerse was used up until 2011 when Creation Engine was created using mainly the C++ programing language. The engine used some of the same code as Gamebryo and with similar features but also improved many parts of it. Creation Engine's first was used for the game Skyrim. Bethesda' is known for they're large and detailed game worlds with npcs inhabiting them. Some of the features added allowed the games open world to feel more immersive, More foilage was possible, NPC interaction and behavior was modified to let them live out day to day activities, have a schedule and places to go, react accordingly to certain situations. The NPC's also could now give quests to the player based on locations not yet explored.
Over the years and with each new game released including Fallout 4 and Fallout 76 even more features were added such as a physically based renderer with volumetric lighting, the ability to paint detailed textures on surfaces, character creation that had more intricate abilities.

With all these new features to the engine the bugs and issues were inevitable. The physics system was flawed. Many say the the engine is outdated and should be replaced with a new and better one.
